During your initial visit, we will conduct a thorough medical assessment, review your medical history, and address any specific health concerns. Prepare a list of your current medications and any medical conditions you wish to discuss. To expedite the check-in process, please arrive 10 minutes before your scheduled appointment to complete necessary paperwork.
Primary Care
Please gather any relevant medical records, including recent test results or imaging reports, as well as a list of current medications you are taking. This information helps us understand your health history and tailor our care to meet your needs. Additionally, take a moment to jot down any questions, concerns, symptoms or changes in your health you may have. Please arrive 10 minutes before your first scheduled appointment in order to complete our patient intake form.
